10 Root Causes of Fatigue Most People Miss
Feeling constantly exhausted, even after a full night’s sleep? If your labs are “normal” but you still feel far from your best, it’s time to explore what could really be draining your energy. Here are 10 overlooked root causes of fatigue I see most often in my clinic:
- Adrenal Dysfunction (HPA Axis Dysregulation)
Your adrenal glands help regulate stress through cortisol production. Chronic stress, overworking, poor sleep, or unresolved trauma can dysregulate the hypothalamic-pituitary-adrenal (HPA) axis. You might feel wired but tired, experience energy crashes mid-afternoon, or wake up groggy no matter how early you went to bed. Supporting adrenal health involves regulating stress, using adaptogens, and restoring circadian rhythms.
- Thyroid Imbalance (Despite Normal TSH)
TSH alone doesn’t tell the whole story. If your Free T3 (the active thyroid hormone) is low or Reverse T3 is high, you may feel hypothyroid even if TSH appears “normal.” Symptoms include fatigue, weight gain, cold intolerance, and brain fog. A full thyroid panel including Thyroid antibodies to rule out autoimmunity is essential to uncover subclinical dysfunction and guide appropriate support.
- Mitochondrial Dysfunction
Mitochondria are the powerhouses of your cells, producing ATP (aka-energy). When damaged by inflammation, toxins, nutrient deficiencies, or oxidative stress, these organelles underperform. This leads to deep cellular fatigue. Restoring mitochondrial health requires targeted nutrients like CoQ10, L-carnitine, NAD+, B vitamins, detoxification and antioxidant support.
- Post-Viral Fatigue (e.g., EBV, COVID-19)
After viral infections like Epstein-Barr or COVID-19, many people develop lingering fatigue due to immune dysregulation, mitochondrial stress, and nervous system overload. Referred to as “long-haul” or post-viral fatigue, this condition requires immune modulation, mitochondrial repair, and gut-healing protocols to restore energy.
- Chronic Infections (Stealth Pathogens)
Infections like Lyme disease, Epstein-Barr virus reactivation, parasites, H. pylori, or candida can persist without acute symptoms. These stealth pathogens provoke chronic inflammation, weaken the immune system, and hijack nutrients. Functional lab testing can uncover hidden infections that drain vitality.
- Leaky Gut & Dysbiosis
When the gut lining becomes permeable (“leaky gut”), inflammatory substances and microbes leak into the bloodstream, triggering immune responses that lead to fatigue. Dysbiosis—an imbalance of gut bacteria—can impair digestion, nutrient absorption, neurotransmitter production as well as immunity. Healing the gut is foundational for restoring energy and nutrient absorption.
- Blood Sugar Instability
If your blood sugar rises and crashes throughout the day, your energy will follow suit. Symptoms include shakiness, irritability, brain fog, and exhaustion after meals. Insulin resistance and hypoglycemia are common but often overlooked contributors to fatigue. Balanced meals with protein, fat, and fiber, plus strategic supplementation, can help stabilize energy.
- Toxic Load (Mold, Metals, Chemicals)
Every day we’re exposed to environmental toxins—mold spores, pesticides, heavy metals, plastics, and household chemicals. These can overwhelm detox pathways and disrupt mitochondrial function. Common signs include fatigue, headaches, skin rashes, or brain fog. Supporting the liver, bile flow, lymphatic system, and sweating mechanisms is essential for detoxification.
- Nutrient Deficiencies
Even a “healthy” diet can leave you low in key energy-producing nutrients. Common deficiencies include B12, iron, magnesium and other minerals as well as vitamin D and CoQ10. Poor absorption, restrictive diets, medications, and stress can all deplete nutrient stores. Testing and replenishment of these nutrients can quickly improve energy and mood.
- Poor Sleep Architecture
Quantity doesn’t equal quality. If you’re not reaching restorative deep sleep or REM stages, you’ll still wake up feeling unrefreshed. Sleep disturbances are often related to hormone imbalances, blood sugar issues, or high cortisol at night. Improving sleep hygiene, managing light exposure, and balancing hormones can restore natural sleep rhythms.

If you see yourself in any of these patterns, don’t ignore your fatigue. It’s a signal your body is asking for deeper support. Functional Lab testing goes beyond conventional measures and pinpoints the root cause.
